Referral

Students remain enrolled at their home school/preschool whilst attending the Hospital School. Nursing staff and parents/caregivers fill out a referral form prior to teaching staff at the Hospital School working with students. Sibling/outpatient referral forms can be completed by parents/caregivers online and submitted to the school. Permission slips for Lunch Time Care arrangements also need to be completed for students staying all day.
